Textbook Adoption Form
Please submit a separate requisition form for each course. It is necessary to provide all requested information for each title ordered to ensure correct ordering. Complete the form for courses even if they are 'No Textbooks Required' courses. Remember the Virtual Bookstore will only order 'required' texts.
