Employees must submit 2023 telework request by Dec. 16
It’s time to begin collecting our Teleworking Reporting and Approval for January–December 2023. No action is required if you are not teleworking during 2023. The link is below and on the Human Resources forms webpage.
SHRA and EHRA employees will complete the form and it will automatically be routed to their supervisor for
approval. It will then be forwarded to HR for approval and System Office reporting.
Temporary employees also will complete this form; however, they must insert the name and email address of their supervisor. The form will the be routed to their supervisor and forwarded from the supervisor to HR for approval.
Faculty do not use this form. The HR-supported ADA accommodation process is the one avenue through which faculty
can receive special approval for adjustments to their teaching delivery mode. Contact

The Teleworking Request should be completed by Friday, Dec. 16. Any changes for an approved telework report can be made at any point via a new Dynamic Form.
