Preferred Name Request Form
The following form can be used to add, delete or change a preferred first name on a student record. Please note that we cannot accept requests to add preferred middle or last names. Also, this request will not change the legal name on a student record. The preferred first name will be used in place of the legal first name in places such as a class roster and on the artist email account. This will not change the name that appears on a student transcript or diploma. Changes that need to be made to legal names require documentation that can be brought to the Registrar’s Office on the 2nd floor of the Welcome Center.
