Annual Evaluations

All faculty classifications will be evaluated on an annual basis using the Annual Evaluation form. It is important that the dean provide an annual opportunity to faculty members to discuss their annual evaluation. Multi-year contract faculty in a contract reappointment evaluation, mid 10-year review or rank promotion year are exempt from annual evaluation in that year.

A copy of the Annual Evaluation form (Part I & Part II) should be kept by the school/division and the faculty member for inclusion in the contract reappointment evaluation packet, the mid 10-year review packet and the rank promotion packet.

Adjunct faculty who teach only in the fall term submit Part I of the Annual Evaluation form to the Office of the Dean.

The dean completes Part II of the Annual Evaluation form.

The Office of the Dean provides a copy of Part II to the faculty member and offers the faculty member an opportunity to discuss the evaluation with the dean.

The Office of the Dean submits Part I and Part II to Faculty Affairs for provost review.  Part I and Part II should be saved as one document for each faculty member.

Save as a PDF and Name Document:  Last Name, First Name - Fall 20XX Evaluation.  Upload completed document to Teams.

Multi-year contract faculty and adjunct faculty who teach in both fall and spring terms submit Part I of the Annual Evaluation form to the Office of the Dean.
