Deadline to register for employee flu vaccine is Oct. 24

For employee convenience, Human Resources is hosting an on-site flu shot clinic. The deadline to register for an appointment is Monday, Oct. 24.

The flu vaccine clinic will be 9 a.m.-1 p.m. Thursday, Oct. 27 in Eisenberg Social Hall, Bottom Floor of Hanes Commons Student Center. You must bring your State Health Plan Medical ID card to receive a free flu shot.

How to register for your on-site flu vaccine appointment

1. Go to the CVS flu clinic reservation website.
2. When prompted, enter your birth date, etc.
3. Select you appointment time.
4. Enter your personal information.
5. Select your coverage information type. Note: If you have the State Health Plan, select "I have only non-Medicare insurance" option.
6. Provide prescription insurance details. Note: Required information is on your NC State Health Plan card. See sample card below to find the require information.
