have received their 1095-C at the permanent address UNCSA has on file. If you did
not receive one, please contact Lauretta Baldwin in Human Resources.
Form 1095-C will indicate your name and employer and the months you were eligible
for coverage during the prior calendar year (2023).
Information on Form 1095-C can assist you in determining eligibility for the premium
tax credit. It is optional to have Form 1095-C to file your return.
