Application Instructions

As part of the application, you will need to upload two documents:

• A completed Filmmaking Student Information Form
• A completed Parent Permission Form

Save the blank forms to your local drive with the following filenames:

• LASTNAME-FILM (where lastname is the student's last name) for Filmmaking Student Information Form.
• LASTNAME-ParentPermission (where lastname is the student's last name) for the Parent Permission Form.

Complete the forms offline, save again to your local drive and upload them when you complete the online application. You can complete the forms now, then upload them when the online application opens.

Mail a hard copy of this form to the address on page 1.
